Any of these columns, in any order, with a header row: name, parent mobile, attendance percentage, last test score, previous test score, fee status, days since last contact. Leave out what you do not track. A missing column is simply not checked, never guessed.
Your rules
Switch off any rule you do not want. The weight is how many risk points that rule adds when it fires. The list re-ranks as you change them.
The WhatsApp message
NAME becomes the student's name. REASON_SOFT becomes a gentle description of what came up. REASONS puts in the exact rule text, and ATTENDANCE the percentage. Keep it warm. A parent who feels blamed stops answering the phone.

This is arithmetic, not a prediction
There is no model here and nothing is learned from your data. The tool adds up the weights of the rules you switched on, for the rows you pasted, and prints which rule fired for each student. You can check every number by hand.
So read it for what it is. A student can trip none of these rules and still leave next month, for a reason no column of yours records: a parent's job moved, a friend switched institutes, a subject stopped making sense. And a student can trip three rules and be perfectly fine. The list tells you who to talk to, not what will happen.
